Role Overview
Mehermedics is seeking highly qualified Clinical Pharmacists to join our hospital partner network. In this technically demanding role you will collaborate with multidisciplinary medical teams to optimise pharmacotherapy for complex and high-risk patients. You will be at the forefront of medication safety, antimicrobial stewardship, therapeutic drug monitoring and patient counselling. A Masters-level qualification and active pharmacist registration are essential.
Key Responsibilities
• Review and optimise medication regimens for hospitalised and outpatient populations
• Participate in clinical ward rounds, providing pharmacological expertise to the medical team
• Lead antimicrobial stewardship initiatives and monitor compliance with prescribing guidelines
• Perform therapeutic drug monitoring and dosage adjustments for high-risk medications
• Provide patient education and medication reconciliation at transitions of care
• Supervise and mentor junior pharmacy staff and pharmacy students
Requirements & Qualifications
• Master of Pharmacy (MPharm), PharmD or equivalent postgraduate qualification
• Active pharmacist registration in the country of placement or commitment to obtain prior to start
• Minimum 3 years hospital or clinical pharmacy experience post-registration
• In-depth knowledge of pharmacokinetics, pharmacodynamics and evidence-based therapeutics
• Experience with electronic prescribing systems and clinical decision support tools
