Role Overview
Mehermedics is seeking Geriatric Care Specialists to join our network of home health agencies. You will provide expert clinical and psychosocial support to elderly patients with complex, multiple co-morbidities, working closely with families, general practitioners and specialist consultants to deliver truly person-centred care. Your expertise in ageing-related conditions will be pivotal in improving patient outcomes and quality of life.
Key Responsibilities
• Conduct comprehensive geriatric assessments covering medical, functional and cognitive dimensions
• Develop and oversee evidence-based care plans tailored to older adults with complex needs
• Manage conditions common in elderly populations including dementia, falls, polypharmacy and frailty
• Provide specialist guidance to CNAs, PSWs and family caregivers
• Coordinate with GPs, physiotherapists and community services for holistic care delivery
• Identify and escalate safeguarding concerns promptly and appropriately
Requirements & Qualifications
• Registered Nurse with post-registration qualifications or demonstrated experience in geriatrics
• Valid nursing license in the country of placement
• Strong understanding of ageing-related conditions and palliative care principles
• Demonstrated compassion, patience and cultural competence
• Experience working with multicultural patient populations is highly regarded
