Key Responsibilities
• Deliver personal care according to each client's individual support plan
• Assist with mobility, transfers and use of assistive devices
• Prepare nutritious meals and monitor dietary intake
• Administer medications under the direction of registered nursing staff
• Maintain detailed and accurate client documentation
• Liaise with supervising nurses, family members and community care coordinators
Requirements & Qualifications
• PSW certificate or equivalent qualification
• Valid personal support worker or home care aide licence
• Experience working with elderly or cognitively impaired clients
• Strong organisational and communication skills
• Ability to build trusting relationships with clients and families
